FULL JOB DESCRIPTION
We are seeking dynamic and self-driven individuals to join our team as a Community Liaison. As a Community Liaison, you will develop and nurture community-based relationships. The purpose of this job is to communicate, network, build strong relationships, educate and follow-up with referral sources in the company's regions. This position requires a great deal of time to be spent in the community and regional environment. The Community Liaison will be responsible for organizing and implementing referral development, marketing and assessment programs to assure awareness and a positive community image. Must be able to work independently, set and track goals and develop an effective community outreach program. This is a rewarding opportunity to make a positive impact on the lives of others.

PHYSICAL DEMANDS:

The employee is typically required to sit; walk; frequently stands, occasionally required to climb or balance; and stoop, kneel, crouch or crawl. The employee must frequently lift and/or move up to 10 pounds and occasionally lift and/or move up to 50 pounds.
TRAVEL:
80% of travel is required between the branch and work site locations. Overnight travel may be required for continuing education and meetings at the corporate office.
Accountable to:
Adorn Health Care Executive Director.
Work Hours:
The position is flexible and will require the candidate to work independently approximately 20-34 hours per week.
